ONLY ONSITE REGISTRATION IS POSSIBLE.

Online registration

All participants are kindly recommended to register preferably via the Online Registration Form. Other registration possibilities are also available below. Please read the instructions on the Internet carefully.

Registration form

If you have no access to the Internet, please send the Registration Form by 25th July 2008 (reduced registration fee) to the Conference Department in Prague. Registration Form is available in PDF form at at the website (to be faxed/mailed to CZECH-IN).

Other registration possibilities

A text version of the Registration Form or Accommodation Form can be downloaded on Conference website. The Registration or Accommodation Form in PDF format is to be used for registration by fax or airmail only. Each participant submitting the Registration Form by fax or airmail will receive a PRN (Participant Registration Number) by e-mail to include in details of payment. Please do not send any payments without PRN included in details of payment. Such payments cannot be identified. All participants and accompanying persons are kindly requested to register in advance by completing and submitting the Registration Form with the appropriate fees.

If your payment is not received before deadline dates 1st July, 15th August 2008 the late fee will be automatically charged.

Please note:

Name changes in already submitted registration is only possible until 25 August 2008. Afterwards, the change will be considered as new registration.

To apply for the EDTNA/ERCA members registration fees, you must be a member in good standing, that is to say that your Membership dues must have been paid before 1st July 2008 (see page 16). After 1st July 2008 you may not apply for members registration fees if you are not a current member. After 10 August 2008 you are kindly requested to make all payments by credit card exclusively or to register on-site. Payment by bank trasfer is no longer available. 

After 1st September 2008 pre-registrations will NOT be accepted any longer by the Conference Department BUT you will have the possibility to register on site.

Registration fee includes:

  • Admission to scientific sessions, exhibition and poster area
  • Conference bag including one copy of Final programme and Abstract book
  • Coffee breaks
  • Admission to Opening ceremony and Welcome reception at Prague Congress Centre

Important
Same as last year, all non-member participants will be offered electronic membership of the association.

Accompanying person fee includes:

  • Access to exhibition Opening ceremony
  • Welcome reception at the Prague Congress Centre
  • Guided tour (Grand Toour of Prague, 8 September 2008, 13:00-17:00)

All Accompanying Persons are asked to re-confirm attendance on this tour by mail to Conference Deparment ( This e-mail address is being protected from spam bots, you need JavaScript enabled to view it ), then they will receive a voucher for this tour with conference badge. 
Please note that Accompanying personś are not allowed  in the scientific sessions and in the exhibition hall.

Confirmation

Upon receipt of the correct registration fee (clear of bank charges), each participant will receive a confirmation of registration. Please bring this confirmation to the registration desk at the Prague Congress Centre as proof of your registration. Confirmation will be sent upon receipt of payment. Please note that registration for the various tours and events will be confirmed upon payment on “a first come, first served” basis, and numbers may be restricted.

Payment

All fees for the Symposium registration should be paid in EURO (EUR), free of all bank charges - clearly stating the participant‘s name and PRN (Participants Registration Number).

    With reference to the worldwide general terms please be aware that your credit card will be charged in currency valid in country of the origin, i.e. Czech Crowns (CZK) and hereinafter converted back to the currency of your account which may result in slightly higher total charge than stated in EUR .

    the actual charge will be made in CZK according to the daily exchange rate of Czech National Bank (www.cnb.cz)

Responsibility

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the Conference be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. With registration, the participant accepts this proviso.

Cancellation / Refund Policy

All cancellations must be notified in writing (by mail, fax or email) to the Conference Secretariat according to the following conditions. Refunds will be processed AFTER the Conference. Requests for refunds made after 5th October 2008 will not be considered.

Please note, that cancellation received:

  • up to 15th June 2008 - 100 EUR will be withheld for administrative fees
  • between 16th June 2008 and 15th August 2008 reimbursement of 50% of the fees paid
  • after 15thAugust 2008 no refund can be processed

Registrants may transfer their registration to a colleague at any time. For any change of name a fee of 30 EUR will be charged.
All registrations received at the Conference Secretariat CZECH-IN, by 25th August 2008 will be included in the list of participants.

All documents including registration packs (conference bag, personal name badge, abstract book, final programme) will be handed out to registered participants during the following opening hours:

Congress Centre Accessibility

Please note that access to the Congress Centre (Exhibition & Poster area and Session Halls) is possible only with valid registration to the conference.
